PACE Southeast Michigan is seeking a licensed Physical Therapist Assistant to support rehabilitation for frail or elderly participants. You will deliver modalities, exercise, and education under supervision, and help coordinate care with the IDT. In this on-site role, you will document care in the EMR, assist with DME management, and advance the plan of care while ensuring participant safety and quality standards.
