What You'll Do

• Evaluate patients and create individualized treatment plans
• Deliver skilled physical therapy services in the home setting
• Partner with physicians, nurses, and interdisciplinary team members
• Educate patients and caregivers on treatment goals and home exercise programs
• Complete timely and accurate clinical documentation

What You'll Bring

• Current Oregon Physical Therapist License
• Valid driver's license and reliable transportation
• Home health experience preferred, but not required
• Strong communication and clinical skills
• A passion for helping patients achieve their goals
