Job Description

Job Summary:
Seeking a skilled RN Case Manager to oversee patient care in a home health setting. The role involves managing multiple cases, documenting thoroughly, and ensuring quality patient outcomes.

Responsibilities:

• Coordinate and deliver comprehensive home health nursing care to assigned patients.
• Manage a high volume of cases, typically seeing 5-6 patients daily, with a focus on quality and efficiency.
• Complete and maintain accurate OASIS assessments and clinical documentation.
• Provide wound care and other specialized nursing services as needed.
• Collaborate with interdisciplinary teams to develop and update individualized care plans.
• Ensure compliance with all home health regulations and agency policies.

Qualifications:

• Valid RN license with active registration.
• Minimum of 2 years recent home health experience.
• Proven case management and OASIS documentation skills.
• Experience in wound care preferred.
• Ability to manage a high-volume caseload with efficiency and attention to detail.
• Strong communication and organizational skills.
