Travel Physical Therapist

Travel Physical Therapist will provide home-based care for a 13-week assignment in Jackson, California. The role offers up to $3,380 per week, independent patient evaluations, and the chance to improve mobility for residents in their homes. You will collaborate with a multidisciplinary team and manage a flexible schedule.

Key Responsibilities:

• Evaluate patients in their homes and develop individualized treatment plans
• Provide skilled physical therapy services to improve strength, mobility, balance, and functional independence
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Monitor patient progress and adjust treatment plans as appropriate
• Coordinate care with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team
• Complete timely and accurate electronic documentation
• Ensure compliance with home health regulations and agency policies

Qualifications:

• Doctor of Physical Therapy (DPT) or Bachelor's/Master's Degree in Physical Therapy from an accredited program
• Current California Physical Therapist License (or eligibility)
• Current BLS Certification
• Valid driver's license and reliable transportation
• Excellent communication, organization, and time management skills
• Ability to work independently while managing a flexible daily schedule

Preferred Skills:
