Do you have a passion for wound care and helping residents heal? Willowcreek Wellness & Rehabilitation is looking for a compassionate Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team as a Wound Care Nurse / Treatment Nurse .

In this role, you'll work closely with physicians, therapists, and the interdisciplinary care team to assess wounds, develop treatment plans, prevent skin breakdown, and improve resident outcomes. As our Treatment Nurse / Wound Care Nurse , you will:

• Complete comprehensive skin and wound assessments

• Develop and implement individualized wound treatment plans

• Perform wound care treatments and dressing changes

• Monitor wound healing and modify treatments as appropriate

• Lead weekly wound rounds with the interdisciplinary team

• Coordinate physician communication and obtain treatment orders

• Maintain accurate wound documentation and measurements

• Monitor residents for pressure injuries and skin integrity issues

• Educate nursing staff on wound prevention and skin management

• Ensure compliance with CMS, state, and infection prevention guidelines

• Coordinate wound care supplies and treatment needs

• Participate in Quality Assurance and Performance Improvement (QAPI)

• Current RN or LPN license in the state

• Experience in skilled nursing, rehabilitation, long-term care, or wound care preferred

• Strong assessment and documentation skills

• Knowledge of pressure injury prevention and wound healing

• Current CPR/BLS Certification

• Excellent communication and teamwork skills

• Passion for improving resident outcomes

Experience in any of the following is a plus:

• Wound Care

• Treatment Nurse

• Skilled Nursing Facility (SNF)

• Rehabilitation

• Long-Term Care (LTC)

• Subacute Care

• Post-Acute Care

• Geriatric Nursing

• Charge Nurse

• Infection Prevention
