Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 Weeks Shift: Day 4x10-Hour Hours per Shift: 10 Experience: 1 year License: RN License required Certifications: BLS preferred, ACM Certification preferred Must-Have: - AK RN License in hand for Auto Offer - Epic experience - Acute Care experience Description: The Case Manager RN is responsible for discharge planning. Must possess an AK RN License for an auto offer. Epic experience is mandatory. Acute Care experience is required. Work schedule involves four 10-hour shifts. Availability to work weekends is necessary, every other weekend or four weekend days in a four-week schedule. There is no on-call requirement for this position. Candidates may be floated within the scope to meet facility needs. No accommodation for block scheduling is available. Local candidates are not accepted; addresses must be at least 50 miles from the facility. Only one time off request is permitted per 13-week assignment of five days or less. Time off during holidays is not allowed for Auto Offer. Guaranteed hours are 70 hours per two weeks, with potential for call-off for one shift. The facility is an acute hospital with a total of 407 staffed beds. Interdisciplinary support includes physical therapy, respiratory services, social services, and rapid response teams. Familiarity with Microsoft Office/Teams preferred. Flexibility with working hours and schedules is vital to meet departmental needs. Candidates must be willing to work two out of the three major holidays: Thanksgiving, Christmas, and New Year. References covering the last year of experience are required before starting the assignment. A commitment to comply with immunization and health safety standards is expected.
