By working as a home health nurse, you help keep patients out of the hospital and in their homes where they are most comfortable. Total Rewards: A comprehensive set of pay and benefits designed to support your well-being, professional growth, and work-life balance.How You Will Make An ImpactDeliver and document professional nursing care in home settings using the nursing process in accordance with North Carolina regulations and agency standards.Conduct comprehensive assessments, develop individualized care plans, and collaborate with interdisciplinary teams to deliver effective care.Educate patients and families on disease processes, medications, and care plans to support outcomes and reduce hospital readmissions.Coordinate care including case management responsibilities, discharge planning, and OASIS assessments tied to reimbursement and quality outcomes.Ensure compliance with regulatory, safety, and quality standards while maintaining accurate and timely documentation.What You Will NeedAssociate Degree or Diploma in Nursing required; BSN encouraged after two years of service.Current RN license in North Carolina or compact state.CPR certification (American Heart Association).Valid driver's license and current automobile insurance coverage.1–2 years of med-surgical, critical care, or home health experience preferred.Knowledge of Medicare and Medicaid regulations, nursing scope of practice, and infection control standards.Strong assessment, critical thinking, communication, and documentation skills with the ability to work independently in home settings.Duke is an Equal Opportunity Employer committed to providing employment opportunity without regard to an individual's age, color, disability, gender, gender expression, gender identity, genetic information, national origin, race, religion, sex (including pregnancy and pregnancy related conditions), sexual orientation or military status.Duke aspires to create a community built on collaboration, innovation, creativity, and belonging.
