Case Manager - PRN

Overview Join to apply for the Case Manager - PRN role at Reunion Rehabilitation Hospitals Position Summary: The Case Manager, in collaboration with the physician, provides individual program management for each patient to ensure the patient\'s progression through the continuum of care in a manner that achieves the desired clinical and financial outcomes. Monitors and manages clinical and financial coordination of treatment plan of assigned patients to ensure timely, cost-effective, individualized service delivery. Works with rehabilitation patients with various disabilities including, but not limited to: spinal cord injury, brain injury, cerebrovascular accident, amputation, neurologic disorders, orthopedic conditions, and arthritis. Coordinates length of stay management within Medicare (CMS) guidelines and 60% compliance threshold. Responsibilities and requirements are described below. Responsibilities Coordinate and manage individual patient programs in collaboration with the treating physician to support progression through the continuum of care. Monitor clinical and financial aspects of treatment plans to ensure timely, cost-effective, and individualized service delivery. Support length of stay management within CMS guidelines and achieve the specified compliance threshold. Work with rehabilitation patients with various disabilities as listed above. Qualifications Current state licensure as an RN, SW, PT, OT, or SLP. Minimum of 3 years of healthcare and clinical experience in a hospital setting. Desired Qualifications 1 year medical rehabilitation experience. Certification in case management preferred. Knowledge, Skills and Abilities Excellent communication, negotiation, and conflict resolution skills. Knowledge of reimbursement systems preferred. Excellent verbal and written communication skills. Strong organizational, time management and prioritization skills. Strong analytical and critical thinking skills. Detail-oriented and able to meet strict time frames.
