Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, families, and groups to determine their mental health needs and appropriate treatment plans.

• Implement graduate-level clinical social work or counseling interventions, utilizing a variety of therapeutic modalities tailored to client requirements.

• Facilitate initial patient assessments and collaborate with interdisciplinary teams to develop and adjust individualized care plans in regular meetings such as patient rounds.

• Provide ethical and timely documentation of patient progress and treatment plans, adhering to the standards set by the organization and state regulations.

• Engage in case management tasks, including coordinating care with external hospital and community professionals to ensure effective treatment pathways for clients.

• Utilize Cognitive Behavioral Therapy (CBT) techniques and other evidence-based methods to assist clients in achieving their therapeutic goals.

Qualifications\n

Compensation and Benefits\n
