Physical Therapist (PT) - Home Health

TLC Nursing Associates, Inc.

TLC Nursing Associates, Inc. is seeking a compassionate and skilled Physical Therapist (PT) to provide in-home rehabilitation services to patients recovering from illness, injury, or surgery. The PT will evaluate functional mobility, develop individualized treatment plans, and deliver therapeutic interventions aimed at improving strength, balance, and independence within the home setting.

Job Responsibilities

• Conduct comprehensive in-home physical therapy evaluations and functional assessments.
• Develop and implement individualized treatment plans based on patient goals and clinical findings.
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions.
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques.
• Assess the home environment and recommend modifications or adaptive equipment to enhance safety.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Document evaluations, treatments, and outcomes accurately in the electronic health record (EHR).
• Collaborate with physicians, nurses, and interdisciplinary team members to coordinate care.
• Adhere to all clinical guidelines, safety standards, and regulatory requirements.

Qualifications

• Active Physical Therapist (PT) license in good standing.
• Doctorate or Master's degree in Physical Therapy from an accredited program.
• BLS certification required.
• Previous experience in home health, rehabilitation, or outpatient settings preferred.
• Strong clinical assessment, documentation, and patient education skills.
• Ability to work independently while maintaining high standards of patient care.
• Excellent communication, time management, and organizational abilities.
