Full-Time Home Health Case Manager – Registered Nurse

Full-time Home Health Case Manager – Registered Nurse provides comprehensive, patient-centered care in the home setting. The role coordinates individualized care plans, collaborates with interdisciplinary teams, and ensures quality and regulatory compliance. It offers autonomy and the chance to improve patient outcomes.

Key Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate and manage patient care across the continuum, ensuring quality and efficiency
• Provide skilled nursing care in accordance with physician orders and established protocols
• Monitor patient progress and adjust care plans as necessary
• Educate patients and families on disease management, medications, and care techniques
• Collaborate with physicians, therapists, social workers, and other healthcare professionals
• Maintain accurate and timely documentation in compliance with regulatory standards
• Ensure compliance with all state, federal, and agency guidelines

Qualifications:

• Active Registered Nurse (RN) license in Idaho or compact license
• Associate or Bachelor of Science in Nursing (ASN or BSN) required
• Minimum of 1–2 years of nursing experience
• Current Basic Life Support (BLS) certification
• Valid driver's license and reliable transportation

Preferred Skills:

• Home health or case management experience (preferred)
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage time effectively
• Experience with electronic medical records (EMR) systems
• Knowledge of Medicare/Medicaid regulations in home health
