OTR | Licensed Occupational Therapist

We are seeking a compassionate and skilled Licensed Occupational Therapist to join our clinical team in the Midwest region. In this role, you will evaluate, treat, and monitor patients while providing ongoing assessments to support their rehabilitation journey. You will develop individualized treatment plans, maintain thorough documentation, and collaborate closely with patients, families, caregivers, and healthcare providers to ensure comprehensive care.

Essential Job Functions

• Conduct patient evaluations and develop treatment plans using approved procedures.
• Communicate treatment goals and progress effectively to patients, families, and care teams.
• Prepare timely documentation including evaluations, progress notes, discharge summaries, and care plans in compliance with federal and state regulations.
• Collaborate with rehabilitation team members to coordinate patient care and select appropriate treatment programs.
• Create and implement home programs to support patient performance in their environment.
• Develop evidence-based treatment protocols and manage referrals from licensed healthcare professionals.
• Supervise occupational therapy assistants and support staff, ensuring adherence to regulations and quality standards.
• Assist with orientation and training of new staff, volunteers, and students.
• Maintain equipment and monitor supply inventory to support department operations.
• Contribute to quality assurance initiatives and complete required facility reports.
• Adhere to all safety, confidentiality, and compliance policies, including HIPAA regulations.
• Maintain positive, professional relationships with patients, families, caregivers, and healthcare team members.

Education and Experience

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy (OTD).
• Current Occupational Therapy board certification and licensure as required by state regulations.
• Basic Life Support (BLS) certification from the American Heart Association.
• Completion of Mandatory Reporter training within six months of hire.
