About the Role:
As a Registered Nurse (RN) / Licensed Vocational Nurse (LVN) at Avir at Vernon, you will play a vital role in delivering high-quality, patient-centered care to our residents. Your expertise and dedication will help us achieve our mission of improving lives. Your responsibilities will include:
• Providing direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
• Developing and implementing individualized care plans in collaboration with the interdisciplinary team
• Maintaining accurate and up-to-date records and reporting changes in patient status to the healthcare team
• Participating in quality improvement initiatives to enhance the overall care experience
• Collaborating with other healthcare professionals to ensure seamless transitions and optimal care outcomes
