How You'll Make An Impact In This Role

Coordinate the interdisciplinary plan of care across the continuum from admission to discharge

Integrate patient and family preferences while monitoring care through assessments and records

Advocate for resources, remove barriers, and maintain ongoing dialog with supervisors and providers

Act as a resource for documentation accuracy, service appropriateness, and post-discharge options

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

• One or more of the following required:
• Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
• Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
• Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
• Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
• Education:
• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
