Setting: Skilled Nursing Facility / Long-Term Care
Shift: Days
Shift Time: 7:00 AM – 7:30 PM
Shift Length: 12 hours
Contract Length: 12 weeks

Job Description

We are seeking a reliable and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care to residents in a skilled nursing and long-term care environment. The selected candidate must be comfortable working at least three 12-hour shifts per week and should have flexibility to work weekends based on facility needs.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and long-term care setting.
• Administer medications and treatments according to physician orders and facility policies.
• Monitor residents and promptly report changes in condition.
• Perform nursing assessments and accurately document patient care.
• Follow individualized care plans and facility procedures.
• Coordinate resident care with CNAs, RNs, physicians, and other healthcare professionals.
• Maintain accurate and timely clinical documentation.
• Follow infection-control and safety standards.
• Provide compassionate, respectful, and resident-centered care.
• Communicate effectively with residents, families, and members of the healthcare team.

Requirements

• Current and active LPN license required.
• Previous skilled nursing or long-term care experience required.
• Reliable and dependable work history.
• Must be able to work at least three 12-hour shifts per week.
• Must be flexible to work weekends, potentially every weekend, based on facility needs.
• Strong clinical, communication, organizational, and teamwork skills.
• Ability to work effectively in a fast-paced skilled nursing environment.
