Care Manager & Community Outreach Lead

A community care organization is seeking a Care Management professional in Fort Lauderdale to work with clients and their families. Responsibilities include developing care plans, engaging with community professionals, and providing support for home care services. Candidates should have a Bachelor's or Master's degree in a healthcare-related field, with at least two years of experience in care management preferred.
