Registered Nurse Wound Care

Job Summary:
The Wound, Ostomy and Continence Nurse provides specialized, compassionate care for patients with complex wound, ostomy, and continence needs. Under the supervision of the Wound and Ostomy Manager, the nurse implements evidence-based interventions to promote healing and optimize patient outcomes.

Responsibilities:

• Conduct comprehensive assessments focused on wound, ostomy, and continence care, developing individualized treatment plans.
• Assist in the development, revision, and evaluation of standards of care for wound and ostomy management.
• Monitor and document patient responses to treatments, ensuring timely adjustments and evaluations of care quality.
• Collaborate with the healthcare team to coordinate patient care and make appropriate referrals.
• Educate patients and families on disease management, prevention strategies, and self-care techniques.
• Participate in quality improvement initiatives, including incidence and prevalence studies and staff education programs.

Qualifications:

• Valid NYS Registered Nurse license.
• Certified Wound Ostomy Nurse (CWON), Certified Wound Care Nurse (CWCN), Certified Wound Specialist (CWS), or Wound Care Certified (WCC).
• Three to five years of healthcare experience, including clinical wound care expertise.
• Proficiency in electronic health record systems, healthcare documentation, and billing platforms.
• Strong interpersonal and communication skills.
