Type of privileges clinic/hospital/both: Hospital only Setting: Inpatient Outpatient or Both: Both The outpatient portion is the ACU If Inpatient # of beds: 122 Rounding admissions or none: Both Patient demographics: 18 Average patients per day: 0-5 Required Procedures: Standard Interventional Cardiology procedures Required clinical activity in the last 24 months case logs: Applicant must provide documentation case list of provision of surgical services representative of the scope and complexity of the privileges requested during the previous 24 months. Case list must include diagnosis or procedure and date and facility where performed. Will provider work solo? If not other providers specialties working with them: Yes Licensed Candidate: Yes/No: Yes candidate must have a Washington state license in hand at time of present. Required certifications Must be from American Heart Association or American Red Cross as applicable: BLS and ACLS EMR: EPIC Shift Info: Day 5x9-Hour 08:00 - 17:00
