This role involves managing client caseloads within assigned service programs. Responsibilities include conducting client intakes, assessing needs, and developing care plans to promote client independence by overcoming barriers. You will build strong client relationships, coordinate support with other service providers, and make referrals to internal and external resources. Additionally, you will facilitate client team meetings and ensure follow-up to achieve client goals.
