Our 192 bed facility is currently undergoing a major renovation, including construction of a state - of-the-art 44bed behavioral and dementiacare unit, enhancing our ability to provide specialized services to the Phoenix and Ahwatukee communities. We proudly maintain a CMS 5-Star Quality Measure rating and foster a supportive, collaborative clinical environment focused on patient-centered success. What You'll Do

- Perform skilled PT evaluations and provide evidence-based treatment
- Develop individualized care plans to increase function, safety, and mobility
- Document patient progress using NetHealth
- Collaborate closely with nursing and IDT using PointClickCare
- Educate patients, families, and caregivers on therapy strategies
- 1 other Full-Time PT
- Multiple Full-Time PTAs
- 2 Full-Time OTs & multiple Full-Time COTAs
- Full-Time onsite SLP A strong and fully staffed in-house therapy department — no contracting or travel required
- Arizona PT license (or eligible prior to start date)
- Strong communication and teamwork skills
- A passion for working with the geriatric and memory care population
