The incumbent will deliver patient-centered care, ensuring safety, regulatory compliance, and measurable progress toward rehabilitation goals for geriatric populations. While serving in a travel capacity, the clinician will work across patient homes, office settings, and community environments as appropriate. Key Highlights

Responsibilities

• Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans
• Provide therapeutic interventions to improve mobility, strength, pain control, and functional independence
• Supervise and coordinate care with Physical Therapist Assistants as applicable
• Collaborate with an interdisciplinary team to coordinate patient-centered care and discharge planning
• Educate patients and caregivers on safe mobility techniques, equipment use, and home exercise programs
• Document care, monitor outcomes, and adjust treatment plans under medical direction and organizational policies

Requirements

• Active Physical Therapist licensure in North Dakota
• Current driver’s license and access to a vehicle to travel to patient homes
• 2 years of prior PT experience strongly preferred
• Ability to work in home health, outpatient, and office/community settings as assigned
• Willingness to travel within assignment requirements and to coordinate care with the care team
