We are seeking a dedicated Physician to provide comprehensive outpatient medical care focused on continuity of care, disease prevention, health maintenance, and chronic condition management. This role serves as a primary healthcare provider, delivering high-quality patient care while collaborating with multidisciplinary healthcare teams to improve patient outcomes and experiences. The ideal candidate is passionate about patient-centered care, clinical excellence, and supporting the overall health and wellness of the populations they serve.

Responsibilities

  • Provide comprehensive outpatient medical care and treatment
  • Evaluate, diagnose, and manage acute and chronic health conditions
  • Maintain accurate and complete patient medical histories and records
  • Develop individualized treatment plans and monitor patient progress
  • Promote preventive care, wellness initiatives, and disease prevention strategies
  • Educate patients on medical conditions, treatment options, and healthy lifestyle choices
  • Coordinate care and refer patients to specialists when appropriate
  • Review and interpret diagnostic testing and laboratory results
  • Collaborate with healthcare teams to deliver coordinated, high-quality patient care
  • Support initiatives focused on improving patient outcomes and consumer experiences
  • Exercise independent clinical judgment in evaluating and treating patients

Required Qualifications

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical school
  • Active and unrestricted medical license in the applicable state
  • Board Certification in: Family Medicine, Internal Medicine, or Geriatric Medicine
  • Minimum of 5 years of clinical experience
  • Strong knowledge of outpatient and primary care medicine
  • Excellent clinical decision-making, communication, and interpersonal skills
  • Ability to work independently and manage complex patient care situations
  • Ability to successfully complete required Tuberculosis (TB) screening for patient-facing healthcare roles
  • Commitment to delivering exceptional patient care and improving healthcare experiences

Preferred Qualifications

  • Internal Medicine specialty training
  • Experience in value-based care, preventive medicine, or population health management
  • Experience managing diverse patient populations
  • Strong collaboration and leadership skills within multidisciplinary care teams
