Key Responsibilities

• Provide direct physical therapy services to patients in a skilled nursing facility setting
• Evaluate patients and develop individualized treatment plans to address functional deficits
• Implement and progress therapeutic interventions in alignment with patient goals
• Collaborate with interdisciplinary care team to coordinate patient care
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapist license required
• Skilled Nursing Facility (SNF) experience required
• Ability to start as soon as possible
• Must adhere to a 14-day notice policy

Preferred Experience / Skills

• Prior travel therapy experience
• Experience working in a fast-paced, high-census SNF environment
