As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Dispensing site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours. 3 positions open. Positions are located in Bryan, Camden, and Long Counties Job duties require home visitation services to medically diagnosed high-risk pregnant, post-partum and inter-conceptual women. They will provide nurse case management to refer for support, education and community resources. Target populations will be provided outreach to reduce disparities in birth outcomes. General clinical support will be provided in the home. Will communicate with health care providers with clients status updates throughout the program. Must have valid drivers license. PH Nurse 1-Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification. $51,811.56 Preferred: Previous experience in home care and/or medical case management, Newborn and/or Pediatric. Bilingual-Spanish. One year of Labor & Delivery experience OR One year working in a prenatal care clinic or One year experience working with moms and babies.
