Job Description
Title: Care Coordinator

Schedule: 8-hour shifts, 8am - 4:30pm, weekends and weekdays

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40

Requirements:
- At least one year of recent Case Management experience in an acute care setting
- Current state Licensure
- Current CPR recognition

Roles & Responsibilities:
- Coordinate care and services for assigned patients in collaboration with the patient, family, physician, and healthcare team.
- Utilize advanced clinical skills to facilitate patient care including assessment, planning, intervention, and evaluation.
- Evaluate and analyze quality and utilization cases, ensuring optimal clinical and resource outcomes.
- Advocate for patients and families throughout the episode of care, maintaining communication among providers.
- Initiate appropriate clinical pathways based on admission diagnosis.
- Serve as a resource for utilization management purposes.
