What You'll Do

Under the supervision of a licensed Physical Therapist, you will:

• Implement individualized treatment plans designed to improve strength, mobility, balance, and function.
• Guide patients through prescribed therapeutic exercises and activities.
• Assist with gait and mobility training using walkers, crutches, braces, and other assistive devices.
• Provide therapeutic modalities as prescribed.
• Monitor patient progress and communicate significant changes to the supervising PT.
• Educate patients and caregivers on home exercise programs and strategies to promote continued progress.
• Assist patients with safe transfers and mobility.
• Complete accurate and timely clinical documentation.
• Participate in quality assurance activities, team collaboration, and ongoing training

What We're Looking For

• Current Physical Therapist Assistant (PTA) license in the state of practice.
• Graduate of an accredited PTA program.
• Minimum 2 years of PTA experience preferred; home health experience is a plus.
• Current CPR certification.
• Strong communication and interpersonal skills
• Ability to work independently and manage visits effectively.
• Reliable transportation and willingness to travel to patient homes.
• Ability to safely assist with patient transfers and mobility, including patients weighing 150+ lbs.
• Ability to meet all applicable state and company compliance requirements
