Bilingual OB Care Navigator

Community Health Center of Snohomish County is seeking an OB Coordinator to manage pregnancy care tracking from intake to six weeks postpartum. You will review daily reports, coordinate referrals, and support the clinical team to meet care measures in a patient-centered medical home framework. The role requires strong organization, communication and collaboration across teams.
