Physical Therapist (PT)
Join our dynamic team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin, United States! As a Physical Therapist (PT) , you will play a vital role in helping our patients recover and regain mobility, strength, and independence.
About the Role:
As a Physical Therapist (PT) at Rock River Nursing & Rehab, you will be responsible for providing high-quality, patient-centered care to individuals with a variety of conditions, including chronic illnesses, injuries, and post-operative rehabilitation. You will design and implement individualized treatment plans, conduct evaluations and assessments, and collaborate with our interdisciplinary team to ensure comprehensive care.
Responsibilities:
Conduct patient evaluations and assessments to identify physical therapy goals and objectives
Develop and implement individualized treatment plans to achieve patient goals
Provide direct patient care, including exercises, stretches, and other interventions
Monitor patient progress and adjust treatment plans as needed
Communicate with patients, families, and healthcare teams to ensure continuity of care
Participate in interdisciplinary team meetings to discuss patient care and progress
Maintain accurate and detailed records of patient care and treatment plans
Requirements:
Bachelor's or Master's degree in Physical Therapy
Current licensure as a Physical Therapist
Minimum of 1 year of experience
Knowledge of current best practices in physical therapy and patient care
