MDS Coordinator

Apple Rehab Uncasville is situated in a quiet country residential community. It is a 130‑bed facility specializing in short‑term care, long‑term care, and a dedicated secured memory unit. APRNs are on staff and rehabilitation staff are trained in the LSVT BIG and LSVT LOUD program for patients with Parkinson’s disease and Parkinson‑like disease. The team is long‑standing and active in the community. Job Description 32 hours per week. Use of MDS is required; candidates without prior MDS experience will not be considered. The RN MDS Coordinator leads the MDS department, assists in coordinating discharge planning and assesses short‑term residents. The role maximizes reimbursement, ensures Medicare compliance, and may manage short‑term managed care cases. Responsibilities include: Gathering information, assessing needs, establishing goals, providing interventions and incorporating them into a concise, functional care plan. Coordinating completion of comprehensive assessments by an interdisciplinary team and documenting recommendations in the resident’s written care plan. Ensuring nursing staff are aware of each resident’s care plan and monitoring compliance, revising plans as required. Planning, scheduling, and conducting weekly care plan meetings for all residents per OBRA and state requirements. Completing the MDS with accuracy to secure highest reimbursement and complying with current CMS Mega Rule guidelines. Point‑Click Care experience is a plus. Qualifications Must hold a current state RN license and be a nurse in good standing. Must meet all applicable federal and state licensure requirements. Strong attention to detail, good follow‑through skills, and the ability to prioritize multiple tasks. Ability to instruct others. Knowledgeable of general, rehabilitative, restorative nursing and medical practices, procedures, laws, regulations, and guidelines governing long‑term care.
