Responsibilities Include Completing start‑of‑care and routine nursing visits Assessing patient condition and developing care plans Providing medication and disease‑management education Monitoring progress and preventing hospital readmissions Coordinating care with physicians, therapists, and care team members Identifying changes in condition and communicating appropriately Documenting visits in the electronic medical record Maintaining HIPAA and patient safety standards Training & Support Structured onboarding Ride‑along visits with experienced clinicians EMR training Ongoing clinical mentorship and leadership support What We’re Looking For Active, unrestricted RN license 1+ year of nursing experience (hospital, SNF, rehab, clinic, etc.) Willingness to work independently in the field Reliable vehicle, valid driver’s license, and auto insurance Ability to travel within the Lafayette coverage area Sit, stand, bend, lift and move intermittently and be able to lift 50‑100 lbs.
