A Brief Overview
The Children's Hospital of Philadelphia (CHOP) Pharmacy Residency Program builds on Doctor of Pharmacy (PharmD) education and outcomes to contribute to the development of clinical pharmacists responsible for medication-related care of patients with a wide range of conditions. Residents who successfully complete the PGY1 pharmacy residency are prepared for advanced patient care positions and postgraduate year 2 (PGY2) training. The CHOP Pharmacy Residency PGY1 Program is a one-year training program that begins at the end of June/beginning of July and runs through end of June of the following year. The residency program is accredited by the American Society of Health-System Pharmacists (ASHP). A certificate for completion of the CHOP Pharmacy Residency Program will be conferred to the resident at the completion of the program requirements. The residency is designed to foster clinical competency in pharmacotherapeutics, an understanding of the practical and administrative considerations of providing pharmaceutical care and activities in clinical research and education. The resident will function as an active member of various multidisciplinary medical teams to provide patient care through monitoring of patient's drug therapy for safety, efficacy, and cost effectiveness with emphasis on utilizing pharmacologic and pharmacokinetics principles. The resident will also participate in departmental education, Formulary management, and unit based quality improvement programs, in additional to order processing and production activities. The resident is expected to adhere to CHOP, Department of Pharmacy, and Pharmacy Residency Programs procedures and policies, including, but not limited to, patient safety, mandatory education, confidentiality, attendance, duty hours and moonlighting, leave, licensure, and problem identification and disciplinary action, etc. What you will do
- 1. Patient Care (direct and indirect)
- Provide appropriate, efficacious, safe and cost-effective pharmaceutical care to patients (clinical and operational)
- Deliver appropriate and timely consultative services (drug information, pharmacokinetic assessment, etc.) to physicians, nurses, pharmacists and other health care professionals regarding drug therapy
- Ensure, promote and contribute to maintenance of guidelines and information (e.g. Policies, Standard Operating Procedures, CHOP Formulary) set forth by CHOP Therapeutic Standards Committee (TSC), its subcommittees, and Department of Pharmacy
- 2. Education
- Advise and counsel (when applicable) patients and caregivers on the use of various medications
- Provide on-demand and live and/or enduring education to Department of Pharmacy or other members of multidisciplinary care team
- Participate in the training (didactic or experiential) of pharmacy students
- 3. Research
- Participate in research activities that will improve the care of patients and/or advance the profession of pharmacy
- 4. Quality Improvement
- Act as a liaison for Department of Pharmacy on multidisciplinary Quality improvement teams (unit or department based or enterprise wide)
- 5. Support mission and activities of Department of Pharmacy not covered above
- Communicate issues to supervisors and peers in a timely manner
- Participate in recruitment activities
- Remain current with pharmacy and medical literature as it pertains to pharmacy practice and area of specialization, if applicable
- Doctorate Pharmacy Required
- Basic knowledge of pharmacokinetics calculations, drug literature review, evaluation of pharmacotherapeutics, and computer applications (Word™, PowerPoint™, Excel™).
- Good verbal and written communication skills.
- A clinical resident will be in the position to recommend changes in a patient's drug therapy where they must be able to interpret and evaluate physicians orders for the most appropriate therapy, i.e. assessing for indication, adjusting for organ impairment, deleting therapeutic duplications, assessing cost-effectiveness, preventing unnecessary extended therapy, converting from intravenous to oral therapy, detecting and preventing drug-drug interactions and drug related allergies and recommending formulary substitutions for non-formulary drug orders.
