We are currently looking for an experienced Home Health Psychiatric Registered Nurse to join our team. As a Psych Registered Nurse , you will deliver customized one-on-one care for each patient developed in concert with the patient’s physician. You will have the relative autonomy and authority to utilize your extensive experience, training, and critical problem-solving skills. You will also develop plans of daily care to address the complexities of patient needs, applying the latest evidence-based nursing interventions, skills, and techniques. Registered Nurse duties include but are not limited to: Evaluate patients for appropriateness of admission and verifies the patient’s eligibility for the Medicare home care benefit, including homebound status, at the time of initial assessment visit and comprehensive assessment. Assist in the developing the plan of care and evaluate the outcomes of care on a regular basis, updating the patient’s plan of care as needed. Identifies goals, interventions and expected outcomes, and involves the patient and/or caregivers in the plan of care and any changes in plan depending on ability of patient/caregiver. Educate patients and family members/care givers on plan of care, addressing their questions and concerns. Provide skilled nursing care as outlined in the patient plan of care and supervise LPN and home health aides. Maintain up to date charting on each patient. Participate in case conferences, in-services and training. Other duties and responsibilities as assigned. Requirements Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation Must have at least 1 year of Home Health experience. HCHB Knowledge preferred. Two (2) years experience as a RN. Must possess and maintain valid CPR certification while employed in a clinical role Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
