MAT Care Coordinator: Substance Use Treatment & Outreach

PrimeCare Health seeks a Care Coordinator for Substance Use Disorder in Chicago. The role coordinates MAT-related treatment, engages patients in care, and supports referrals to social and mental health services. Excellent communication and teamwork are essential for success in a fast-paced clinic. Responsibilities include tracking outcomes, coordinating with PCPs and therapists, and participating in daily huddles to assess progress and adjust plans as needed.
