Continuing Care Coordinator/Discharge Planner Talbott Recovery is currently looking for a full‑time Continuing Care Coordinator/Discharge Planner for our Main Campus on Hunter Road, Atlanta. Essential Duties and Responsibilities Establishes and maintains referral relationships. Contacts referral sources upon admission and confirms patient’s needs and expectations regarding updates/information. Coordinates all communication and information exchange with referral sources. Develops a comprehensive and patient‑focused continuing care plan for assigned patients. Timely and accurate input of contact information into an established system. Accesses, assesses, and reports on requested and/or needed information. Education and Experience Experience in alcohol/substance abuse/12‑Step program. Experience in discharge planning, case management, or continuing care management. Must have a high school diploma or equivalency; bachelor’s degree strongly preferred.
