Licensed Physical Therapist (PT) - In-Home Care

Location: Grand Junction, CO

Assignment Duration: This is a 13-week contract position.

Schedule: Monday - Friday, 8 am - 5 pm

Job Description: We are seeking a licensed Physical Therapist (PT) to deliver in-home physical therapy services to patients in Grand Junction. The Physical Therapist will provide skilled therapy services in accordance with physician orders, payer requirements, and agency policies, contributing to patient mobility, safety, and functional independence through tailored, evidence-based care.

Responsibilities:

• Perform assigned physical therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
• Develop and implement individualized physical therapy plans of care based on patient needs and physician orders.
• Provide skilled interventions such as gait training, balance training, therapeutic exercise, mobility training, and fall prevention.
• Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies.
• Communicate patient progress, concerns, or changes in condition to the interdisciplinary care team.
• Collaborate in the clinical oversight of Physical Therapist Assistants (PTAs), as applicable and permitted.

Required Experience / Certifications / Licensure:

• Current, unrestricted Colorado Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• Valid driver's license and reliable transportation.
• Ability to practice independently in a home-based setting.
• Proficiency or willingness to learn Homecare Homebase (HCHB).
