JOB TITLE: Case Manager
REPORTS TO: Outreach Advocacy Manager
LOCATION: Kane County
HOURS: Part-time, 20 hours per week
PAY: $20 per hour
BENEFITS: PTO, EAP, Group Life Insurance paid 100% by employer, 401k 3% match after six months of employment
JOB SUMMARY:
The Case Manager provides survivor‑centered advocacy and case management services to individuals
experiencing domestic violence and sexual assault. Working in rural community-based settings, the Case
Manager supports clients in crisis by assessing needs, developing client‑driven action plans, providing
safety planning, and connecting survivors to community resources and supports that promote stability
and self‑sufficiency. Services are delivered in person and by phone, using a trauma‑informed and
culturally responsive approach.
The Case Manager builds professional, supportive relationships with clients while maintaining clear
boundaries and honoring client autonomy and self‑determination. This role requires flexibility, strong
organizational and time‑management skills, and the ability to work collaboratively within a
multidisciplinary team. Case Manager is expected to remain current with best practices in victim
advocacy through required training and ongoing professional development.
RESPONSIBILITIES:
● Provides case management services, including intake, needs assessment, safety planning,
development of client-driven action plans, referrals to community resources, and timely
documentation
● Provides court advocacy; assistance with filing protective orders/stalking injunctions, safety
planning and attending court with clients when needed.
● Engage in ongoing case staffing and supervision with program manager
● Provides child care for clients’ children during groups, individual therapy sessions and court
advocacy appointments to support client participation in services
● Integrate financial empowerment education and support into case management services
● The Case Manager will document all client interactions within 24 hours of meeting with the
client
● Assists with outreach events and community activities, including availability to support evening
and weekend events as needed
● Participate in the Hospital Advocate Response Team (HART) on-call rotation, providing crisis
response and support as required
● Additional duties may be assigned as necessary to support evolving program needs, staffing
changes, team coverage, and continuity of services. In these circumstances, a supervisor will
approve these duties to ensure they do not change the core purpose of this position

● Strong organization skills and ability to manage time, documentation and multiple priorities
effectively
● Preferred Bachelor’s degrees in Social Work or other Behavioral Science filed and two years of
experience in human services; or an associate degree in a human services or related field and
three years of experience in human services; or five years of experience in human services
● Ability to work evening and weekends as program needs require
● Strong organizational, time management, and self-direction skills, with the ability to complete
tasks with minimal supervision
● Ability to work independently and collaboratively as part of a multidisciplinary team
● Computer proficiency with word processing, spreadsheets, databases, and email platforms,
including Google Workspace and Microsoft Office applications
● Knowledge of community resources and referral processes, or ability to quickly acquire this
knowledge
● Valid driver’s license, current auto insurance, reliable transportation
● Ability to pass required background checks (BCI) per UT DHHS contract
● Excellent written and verbal communication skills
● Knowledge of and ability to comply with local, state and federal laws pertaining to confidentiality
