Position Part time Registered Nurse Case Manager Anticipated Territory : Greater Sacramento County Schedule: Every Saturday and Sunday full days. Would consider Saturday/Sunday plus 3 week days. What you’ll do Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including Start of Care (SOC) and OASIS Conduct Start of Care (SOC) Admissions - OASIS Conduct follow ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided Why you should come work for us Concierge for Case Managers: Dedicated resources assist with time consuming administrative tasks Flexible work schedules: Create your own schedule and hours Targeted territories. Generous PTO, Medical, Dental, Life Insurance, etc Competitive compensation with achievable incentive bonus program To be a success Current and unrestricted CA RN license Current BLS/CPR Certification Valid CA Driver's License Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired High level of clinical knowledge in home health care is highly preferred Experience with high acuity patients is a plus Experience working with an interdisciplinary team is highly desired Case Management fundamentals is highly desired Previous experience with IV Medication infusions highly preferred Proficiencies initiating intravenous (IV) therapy is a plus Self-Directed Empathetic Critical thinker Outstanding interpersonal skills Leadership skills Organized Compensation This position is compensated on a per-visit basis, with rates varying by visit type. Visit rates range from $90 to $150 per visit , depending on visit type and documentation requirements.
