Weekend Admission Nurse (RN) - Start of Care/Health Care

Position Part time Registered Nurse Case Manager Anticipated Territory : Greater Sacramento County Schedule: Every Saturday and Sunday full days. Would consider Saturday/Sunday plus 3 week days. What you’ll do Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including Start of Care (SOC) and OASIS Conduct Start of Care (SOC) Admissions - OASIS Conduct follow ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients’ conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Initiate and administer IV medication (antibiotics and diuretics) infusions safely and effectively in accordance with established protocols Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided Why you should come work for us Concierge for Case Managers: Dedicated resources assist with time consuming administrative tasks Flexible work schedules: Create your own schedule and hours Targeted territories. Visit rates range from $90 to $150 per visit , depending on visit type and documentation requirements.
