Duties included: Provides quality patient-centered care to adults age 18-65 and older adults age 65+, as assigned by ACOS of Primary Care. Assesses Veterans by utilizing advanced clinical judgment and physical assessment skills. Identifies acute/non-acute/chronic health problems. Ensures that safe, efficient, and high quality healthcare is delivered within the department. Ensures appropriate initial evaluation, treatment, and consultation, as well as documentation, consistent with current VHA Directive and VHNIHCS policies on Emergency Medicine. For admissions to the inpatient units, will ensure a physician-to-physician conversation regarding the need for admission, including diagnoses, assessment and recommendations. Demonstrate sound decision making skills, both clinical and administrative, which bring credit to the profession and serve to enhance the distinctive and contributory role of physicians. Daily care of patients also entails regular interaction with nursing, coordination of consultations and care and facilitation of diagnostic studies and therapeutic procedures. Other vital roles include: communication with patients and their families, patient education, discharge planning and communication with outpatient providers, arranging follow up, preparing discharge instructions, medication reconciliation, and ensuring communication during hand-offs of care. Ensures a sign-out to the oncoming physician to ensure an informed transition of care; coordinate with the admitting physician and consultants to organize diagnostic and treatment plans for all patients pending admission. Maintain appropriate clinical privileges in order to fulfill the duties of assignment within Primary Care Service.
