About the Position

Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 6 months Shift: Day shift Hours per Shift: 8 hours Experience: - Minimum 2 years of Acute Care Case Management or Discharge Planning experience required. - Hospital experience is required; behavioral health experience does not apply. License: - California state license required. - Pending licenses accepted, but must be licensed upon start date. Certifications: - Applicable certifications based on specialty and experience. Must-Have: - Experience in care coordination, discharge planning, and utilization review. - Ability to organize and participate in family meetings. - Competence in arranging durable medical equipment (DME), skilled nursing facility (SNF), and ambulance transport independently. Description: The position involves managing case responsibilities within an acute care facility. Daily patient assignments may involve a caseload of up to 25 patients. Traveling professionals are expected to float between facilities as necessary. Participation in the development of treatment plans is a fundamental duty. Knowledge of medical systems and processes is essential, including familiarity with various insurance models. Experience dealing with diverse patient age groups is crucial. Candidates should understand issues surrounding advanced directives and protective services processes. This role may require additional training related to documentation and compliance standards. Professional appearance is expected, with business attire or scrubs as appropriate. Candidates need to commit to covering every other weekend. Collaboration with interdisciplinary teams, including physical therapy and social services, is essential. References covering recent work experience will be required during the compliance process.
