Physical Therapist

Overview Professional Case Management is seeking a dedicated Physical Therapist to provide in-home rehabilitation services to clients with complex medical needs, including former energy workers and their families. In this role, you will travel to clients’ homes to perform comprehensive physical therapy evaluations, develop individualized treatment plans, and deliver evidence-based interventions focused on improving mobility, function, pain management, and overall quality of life. You will collaborate closely with nurses, physicians, case managers, and caregivers to coordinate care, adjust treatment goals, and ensure safe, effective transitions in the home setting. Responsibilities Assess strength, range of motion, balance, and functional abilities in clients using home-based evaluations. Instruct clients and families in therapeutic exercises, transfer techniques, and the use of assistive devices. Document care in accordance with regulatory and company standards and monitor progress to update plans of care. Educate clients and families on injury prevention, energy conservation, and creating safe home environments. Participate in interdisciplinary case conferences, contribute to care planning, and support a culture of clinical excellence. Travel to clients’ homes as part of a coordinated care team; collaborate with nurses, physicians, case managers, and caregivers to ensure safe, effective transitions.
