Develop, implement, and revise individualized treatment plans; monitor progress and adjust goals as needed. Conduct bedside assessments and therapy, including swallowing safety and feeding strategies; document outcomes. Collaborate with an interdisciplinary team (nursing, therapy, social work) to coordinate care and support discharge planning. Educate residents, families, and care staff on communication strategies, compensatory techniques, and swallowing safety. Maintain accurate documentation in medical records and ensure compliance with facility policies and state regulations.
