Licensed Practical Nurse (LPN) – Home Care | Millersburg, OHPosition DetailsFull-time, Monday-Friday, 8:00 AM - 4:30 PMBased with the Millersburg team, serving the southern portion of the regionExtensive driving is required throughout the workdayParticipation in a weekend rotation (currently every 9th weekend, subject to change)Two holiday shifts per yearMileage reimbursement providedEssential ResponsibilitiesProvide skilled nursing care under the direction and supervision of a Registered Nurse (RN) in accordance with the patient's plan of care and within the LPN scope of practice.Assist with nursing assessments by observing, collecting, and reporting patient information to the RN.Deliver care that addresses the physical, emotional, spiritual, and psychosocial needs of patients and their families.Monitor patient condition, recognize changes in status, and promptly communicate findings to the RN.Administer medications as prescribed using the correct dose, route, and frequency.Educate patients and caregivers on self-care techniques as directed by the RN.Document patient care, observations, and interventions accurately and promptly in the electronic health record.Collaborate with the interdisciplinary team to promote patient comfort, safety, and quality outcomes.Assist other departments and team members as needed to ensure comprehensive patient care.Participate in Continuous Quality Improvement initiatives.Serve as a preceptor for new team members when assigned.QualificationsCurrent Ohio Licensed Practical Nurse (LPN) license in good standingOne year of nursing experience preferredCurrent CPR certificationBasic computer skills for electronic documentationValid driver's license with a safe driving recordState minimum automobile insurance coverageAbility to travel extensively throughout the service area, including daytime, evening, and inclement weather drivingBenefits & PerksCompetitive compensationMedical, dental, and vision insuranceShort-term and long-term disability coverageLife insurancePaid time offRetirement plan with employer matchTuition reimbursementCompany-provided scrubsMileage reimbursementDedicated preceptor for onboarding and ongoing supportProfessional development and career advancement opportunitiesWhy Join Ohio's Hospice?At Ohio's Hospice, our mission is to celebrate the lives of those we serve by providing exceptional care and compassionate support.
