Nurse Practitioner Portland OregonOregon Integrated Health is an Integrated Primary Care Medical Home that is hiring for a Full Time Nurse Practitioner for our Clinic in Salem Oregon. We are a Tier 5 Patient Centered Primary Care Medical Home, Vaccine for Children Program. We have an amazing team and looking forward to our growth in Portland!Our Portland clinic sees a demographic of Medicaid and Medicare individuals that are in need for our expansion with community resources.Our Integrated Medical Home in Salem currently has a team of Family Nurse Practitioners, Naturopathic Physicians, Licensed Professional Counselors, Behavioral Health Clinicians, Chiropractor, Acupuncture, Medical Assistants, Front Desk and community health worker.We utilize telemedicine visits for patients that are at home as well as telehealth visits – when patients may not have access to data or smart phones patients come to our facility and utilize our technology to connect with any of our providers across our 4 locations! Our additional locations in Florence, Eugene and Portland are comprised of Family Nurse Practitioners, Naturopathic Physicians, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture.$8,000 Sign On Bonus, paid first pay period following 90 days of employment with OIHSalary $137,000-$140,000 based on Experience and FTEFlexible with hours and schedulingHealth, Vision, Dental Benefits paid at 50% of Base Plan offered by OIH, eligible after 60 days of employment401K Benefit: OIH matches 100% of your contributions up to 3% of your compensation, plus OIH matches 50% on your deferrals that are greater than 3%, but less than or equal to 5% of your compensation (Eligibility Requirements: 1yr employment, 1000hrs worked, at least 21yrs of age)Eligible for OIH Benefits after 90 Day Probation PeriodState Medical License and DEA 100% reimbursed, completing 90 days of employmentContinuing Education Benefit up to $700 annually, completing 90 days of employment8 Holidays Per Year:7 Clinic Closures, 1 Floating Holiday, eligible immediately upon hirePTO 160 Hours Per Year:Vacation PTO accrues based on time worked, with a maximum annual accrual of 80 hours per FTE 1.0, available for use after 90 days of employmentFrontloaded PTO is uploaded based on time worked, with a maximum annual eligibility of 80 hours per FTE 1.0, available for use after 90 days of employmentProtected Sick Time accrues 1 hour for every 30 hours worked, max use of 40 per year, available immediately as time accrues after first day of employmentJOB DUTIES AND RESPONSIBILITIES:· The Primary Care Provider will provide primary care in an integrated setting with various other specialties at the clinic.· Primary Care Provider will have access to Medical Assistants that will room patients, collect vitals, submit referrals and perform electronic orders as submitted by provider.· Primary Care Provider will have access to Patient Health Navigators that will follow up on ED visits, referrals, complex care patients and quality metric data.· New patient visits are 45 minutes and follow up 30 minutes, Urgent appointments are 15 minutes.
