Key Responsibilities\n

• Assist in program planning and the organization of services to maximize patient care delivery.
\n
• Communicate regularly with physicians and other healthcare team members to report on patient progress, needs, and any concerns that arise during treatment.

• Maintain accurate records including clinical notes and progress updates, adhering to department policies and regulations.
\n
• Collaborate with team members to develop comprehensive treatment plans, including psychosocial assessments and discharge planning tailored to each patient.
\n
• Provide counseling and crisis intervention services to patients, effectively managing and treating emotional and behavioral challenges.
\n
• Consult and coordinate with patients’ families and support systems to ensure a unified approach to treatment and a successful transition during discharge.

Patient Population / Client Focus\n

Your primary focus will be on high-risk patients who are dealing with mental health issues stemming from personal and environmental stressors. These include individuals diagnosed with severe depression, anxiety disorders, and other mental illnesses that affect their daily functioning and quality of life. You will work closely with these patients to empower them in their recovery journey and enhance their ability to engage in their medical care effectively.

Qualifications\n

• A Master’s degree in Social Work, Counseling, Psychology, Marriage and Family Therapy, or a related field.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.

• Minimum of 3 years of experience working in an outpatient setting, with a strong background in mental health.
\n
• Proficient in electronic medical records and capable of using various computer applications such as word processing, spreadsheets, and email systems.
\n
• Exceptional communication skills with a collaborative approach to teamwork.
\n
• Fluent in written and spoken English; additional language skills are a plus.

• Experience working with high-risk populations and in crisis intervention.

• Familiarity with holistic or integrative care models.
