RN Care Manager: Rehab & Discharge Lead

Gaylord Hospital

Gaylord Specialty Healthcare in Wallingford, Connecticut, is seeking a Care Manager to coordinate all health care services for designated patient populations. You will develop discharge plans, assess care quality, and establish communication among providers and stakeholders. The ideal candidate will have at least 2 years of RN experience and hold a current CT RN license.
