Speech Pathologist | Nursing Home

Role Overview The Speech-Language Pathologist evaluates and treats patients with speech, language, cognition, voice, and dysphagia deficits in a home setting. They develop individualized treatment plans, perform assessments, and deliver direct therapy using age-appropriate strategies. The role involves collaborating with interdisciplinary teams, documenting progress, and participating in program development and student internships. Key Responsibilities Conduct assessments and interpret diagnostic tests to determine patient needs. Create and adjust personalized treatment programs based on ongoing evaluations. Provide therapy tailored to patients’ physical, emotional, and intellectual capabilities. Maintain detailed documentation, including evaluation results and progress notes. Communicate effectively with patients, families, and healthcare teams. Participate in departmental program development and supervise clinical internships. Qualifications & Skills Current CCC-SLP certification or active state licensure. Preferred 1-3 years of experience in diverse settings such as hospitals, SNFs, outpatient, or home health. Strong communication, teamwork, and confidentiality skills. Ability to perform job functions with adequate visual acuity and hearing.
