Home Health RN position available for a full-time schedule from Thursday to Monday. We are a community-focused not-for-profit organization dedicated to enhancing the well-being of our community members. We deliver exceptional home health and hospice care to individuals of all backgrounds. Our services ensure access to high-quality care regardless of financial circumstances. We are committed to holistic care, offering a range of health promotion, long-term care, and maternal-and-child health services. This position is responsible for providing individualized, goal-oriented nursing care. Care is delivered using the nursing process in accordance with a physician's order. In addition, this position serves as Case Manager in cases where the RN is the lead discipline. This is a position with rapid response urgency.
