Home Health LPN ContractSupplemental Health Care is hiring experienced Home Health LPNs (1+ year in a Medicare-certified agency) for contracts in O Fallon, Illinois, and nearby areas.Qualifications:Active, unencumbered LPN/LVN license in the state of assignmentCurrent BLS certification1+ year of recent experience (within 3 years) in home health, hospice, or skilled nursingSkilled in wound care, injections, med administration, and vitals under RN supervisionFamiliar with OASIS-E functional domainsClear background check, abuse registry, and driving recordHealth clearance with TB and required immunizationsReliable transportation and valid auto insuranceWilling to travel within the assigned area and adapt to patient needsHome Health Nurse Contract Details:$1,226 - $1,328 per week*13-week contractSHC Home Health Nurses are paid by the hour, not by the visitVisits typically last 30 to 60 minutes depending on the care plan.LPNs are expected to conduct 6–8 visits per day based on task complexity and travel requirements.Travel time is incorporated into productivity planning and may vary based on geographic spread.Key Responsibilities:Provide skilled nursing services under the direction and supervision of an RN.Monitor patient status and immediately report changes to the supervising RN or physician.Accurately document all care provided in compliance with agency policies and state nurse practice acts.We understand home health care is unique and requires the right support for success.
