Responsibilities:

• Promote Elara Carings philosophy, mission statement, and administrative policies to ensure quality of care.
• Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
• Design and implement individualized plans of care based on assessment and incorporate participation by significant others when appropriate.
• Conduct therapy practice within defined standards of care.
• Direct support staff to achieve patients functional goals and maintain detailed documentation of evaluations and treatment plans.
• Provide direction and leadership to staff and act as a community resource to promote physical therapy.

Qualifications:

• Graduate of an APTA-approved physical therapy program.
• Current, unrestricted PT license valid for the state of work.
• Minimum one (1) year of experience in a healthcare facility.
• Dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws.
• Able and willing to travel within branch coverage area (up to 50%).
• Ability to lift, push, pull, and support up to 50 pounds, including positioning or transferring patients.
