Registered Nurse (RN) – Home Health

$5,000 SIGN-ON BONUS | 401(k) MATCH | DIRECTOR OF NURSING GROWTH OPPORTUNITY

Part-Time to Full-Time | Minimum 1 Year RN Experience Required

Serenity Homecare is growing, and we are looking for an experienced, motivated, and compassionate Registered Nurse (RN) to join our team as we expand our home health services.

This is more than just another nursing position. We are looking for an RN who wants to provide excellent one-on-one patient care while also having the opportunity to grow with our agency and potentially advance into a Director of Nursing (DON) leadership role .

This position may begin part-time with the opportunity to transition into full-time employment as our patient census grows .

Responsibilities

• Perform skilled nursing visits in patients' homes
• Complete new patient admissions and recertifications
• Conduct comprehensive nursing assessments
• Develop, implement, review, and update individualized plans of care
• Monitor patient progress and changes in condition
• Provide medication, disease-management, and safety education
• Communicate with physicians, providers, families, and members of the interdisciplinary care team
• Complete accurate and timely clinical documentation
• Assist with coordination of patient care
• Ensure nursing services meet applicable state, federal, payer, and agency requirements
• Participate in quality improvement and clinical oversight as needed
• Help support the continued growth and development of our home health program

Qualifications

• Current, unrestricted Registered Nurse (RN) license in Nebraska
• Minimum of 1 year of professional RN nursing experience required
• Home health experience preferred, but not required
• Strong clinical assessment and critical-thinking skills
• Excellent written and verbal communication skills
• Compassionate, dependable, professional, and organized
• Comfortable working independently in patients' homes
• Strong time-management and documentation skills
• Ability to work with diverse patient populations, including geriatric patients, individuals with developmental disabilities, dementia/Alzheimer's, and pediatric patients when applicable
• Interest in professional growth and nursing leadership
